MPC-HC keeps breaking on CCCP

So I've been running CCCP and MPC-HC forever, but recently after installing some new phone software, my MPC keeps pulling the "black screen, audio only" for every single filetype - MKV, MP4, AVI, FLV.. If I reinstall CCCP, it works fine... for a few hours. If I leave my PC running all day and come home, the problem comes back. I reinstall CCCP (no restarts, no uninstalls) it works fine.
I tried to run insurgent and test render and this is what I get:
An error occured during this Test Render. Please provide the info below to CCCP Staff.
DirectShow Error: 0x80004002: No such interface supported
Haali is installed and such. I'm up and running now, but I'm afraid it will happen again. I also reset the settings, but have to change the MPC-HC output from "System default" to "Haali renderer" to get it to work.

Please see steps in my signature to produce a CCCP Insurgent log while it is not working.
Steps for reinstallation:
Please restart your computer and then reinstall CCCP. Make sure to restart afterwards if asked and install the latest DirectX Runtimes as you will be asked if MPC-HC was set to "System Default".
When asked, let the installer do a reset of settings. Then, go into the CCCP Settings application and tick reset and re-register on the second page and click apply and then yes and then close the CCCP Settings application.

So it started occuring again on .avi files.
I reinstalled and followed your suggestion.
Restart -> Uninstall -> Restart -> Install -> Restart -> Re-register filters. It was not working by this point. I changed from "System Default" back to "Haali Media renderer and it started working again.
What I don't understand, is that WMP works fine, so what "System Default" is MPC using?
1) Default DirectSound Device, Audio Switcher, MPEG-1 Audio Decoder (low merit), Haali Media Splitter (AR), The path of the movie.
