Also what settings are you changing from the defaults exactly?

Vista madness, I could guess most probably VMR7 which is being default in Vista is the cause. Try changing the video renderer to Haali: http://www.cccp-project.net/wiki/index.php?title=FAQ#How_do_I_change_video_renderers.3F
I doubt that could cause such huge mess, and Haali might be pushing it for integrated stuff. So test with Overlay Mixer (and maybe EVR) too.

About the audio thing, I just happened to be using another computer with no audio output. Same video problem occurs (tried on more than 2 computers).

@Lord: default settings when installed. Tried all the video renderers in media player classic.

Just realized that might be an audio error message. The tested computer doesn't have audio.
If you want I can test on one with audio (a few computers here)

It really didn't seem so from your test render. So do reset the settings once more.

Tried all the video renderers in media player classic.
Did you make sure to open the file after changing the video renderer each time too?

And yes, it would be better to test on a system with an audio device too. In theory it shouldn't cause things like this at all, but at least avoids dealing with some random madness. Post an Insurgent log with everything from that system too in this case.

EDIT:
The problem was confirmed, and currently being investigated. In the meantime I would recommend to just use CoreAVC if you have it anyway. And thanks for reporting.

Completely reinstalled settings --> same thing
Tried the renders and re-opened fine --> same thing

Do you still want the insurgent log if it's a confirmed bug?

It's strange that the problem does not occur in the first episode of chrno crusade by KAA, which is also h264 encoded in mkv format.
